In fact, the rampage story of Unit-00 has always been regarded as a major mystery in the EVA series.
This is quite normal because Fang Zheng’s impression of Unit-00 has always been about not rampaging or being on the verge of rampage... In short, except during actual combat where it’s considered normal, Unit-00 always refuses to remain quiet during test phases.

As for Lilith, it’s even more implausible; everyone knows Rei Ayanami’s soul uses Lilith’s. If Unit-00 was also Lilith, how could there be rejection?
Another theory suggests that Unit-00, as the original prototype, has various defects, and the prototype itself has significant issues. This theory builds on the basis of Unit-02, which itself is quite unique, notably Asuka feels her mother’s soul in Unit-02, but in fact, Asuka’s mother also committed suicide without dying anywhere associated with EVA.
But if this explanation holds, there’s a puzzling issue—assuming that Unit-02 is sheltered by Asuka’s mother’s soul, it should similarly resist outsiders boarding it strongly. In fact, when Rei Ayanami attempted to board Unit-01, she was directly rejected.
However, the reality is Unit-02 has always been stable, regardless of who boarded without issues, Asuka faced no issue, Nagisa Kaoru faced no issue, and even Makiha faced no issue—Shinji Ikari initially also entered the Unit-02’s insertion plug together with Asuka, yet no issue was triggered.
Only Rei Ayanami never boarded Unit-02, so couldn’t verify the last theory—that these machines themselves have no defect, but Rei Ayanami herself is mentally unstable, therefore easily provoking rampages and rejection reactions in any EVA she pilots.
Originally, there was almost a chance in the animation for Rei Ayanami to verify if this hypothesis was correct, but unfortunately, Asuka stubbornly refused to let Rei Ayanami board Unit-02, thus, driven by a spirit of scientific rigor, this most probable theory could only remain hypothetical.
If Rei Ayanami experiences a rampage reaction when piloting the "anyone-can-pilot" Unit-02, then the theory would be verified as evidence.
Of course, on the surface, this hypothesis doesn’t seem to hold up, since Rei Ayanami doesn’t display much emotional fluctuation and doesn’t seem like someone who would go berserk. However, the facts have shown that Rei Ayanami has no desire for survival, and even several times has had suicidal thoughts. Even if such a person doesn’t have personality issues, there might be some unspeakable thoughts in their subconscious.
After all, charging directly with an N2 bomb for self-detonation and frequently fighting with tactics of mutual destruction—this is not something someone who appears calm could do.
But now, having seen Unit-00 up close, Fang Zheng has finally pinpointed where the legend of Unit-00’s berserk issue stems from.
Just like Hypothesis No. 2 previously guessed, as the first fully-formed prototype, Unit-00 has a flaw on the soul level.
