The Enhanced Doctor-Chapter 808: Twin Blades in Action
Engaging in a casual conversation with Wang Chao, Liu Banxia managed to resolve some of his internal frustration.
Though he understood, each person harbors their own opinions on every matter. In Liu Banxia's heart, he felt the girl should confront the opposition head-on, making them pay the necessary price.
However, this was mere fantasy; after all, he was merely a doctor and couldn't make decisions for others.
"President Liu, a patient with fall injuries is being sent over immediately. Will you see them?" the dispatch nurse inquired.
"Have the basic details been reported?" Liu Banxia asked.
"Blood pressure 70/90, fracture of the right tibial-fibular neck, suspected internal bleeding," the dispatch nurse replied.
"Alright, I'll handle this patient. Seems this team can't stand to see me idle. One more day to get through, and I'm free," Liu Banxia stated.
The dispatch nurse chuckled; it seemed like she was part of a conspiracy in this. However, it was true they hadn't seen Liu Banxia perform a live rescue in months, leaving it to others instead.
Liu Banxia glanced over, "Su Wenhao, come with me for an internship observation, prepare blood bags." Liu Banxia instructed.
The interns felt a bit excited; witnessing Liu Banxia's medical skills was akin to seeing a legend come to life. If they could observe him rescuing critical patients, they would learn even more.
In less than ten minutes, the rescue vehicle pulled up at the emergency center door.
"Patient, 17 years old, skateboard accident. Blood pressure 70/90, heart rate 120, abdominal hardness, right tibial-fibular neck fracture. Fluid replenishment on the ambulance, right leg tourniquet applied, no family medical history," reported the EMT pushing the patient in.
"Rescue Room One, have you contacted their family?" Liu Banxia asked.
"Contacted on the ambulance, but they work in the development zone and are rushing here," the EMT replied.
"It was quite a fall, it might hurt a bit later, hang in there," Liu Banxia said to the patient.
"Your right leg has started swelling now, I need to perform manual bone setting and a local fasciotomy decompression procedure. Otherwise, the leg could be lost."
The patient wasn't in great shape mentally, only nodding slightly.
With the patient's consent, Liu Banxia moved swiftly, after feeling the deformed part of the patient's leg, he immediately pulled, twisted, and supported, successfully completing the bone setting.
Though this procedure would typically be painful, the patient's reaction wasn't as intense, just a bit of groaning.
"One, two, three, transfer the patient to the bed. Connect ECG, rush blood test, cross-match, open IV line, fluid and blood replenishment. Schedule the operating room, call the orthopedic surgeon," Liu Banxia issued a stream of orders.
"Su Wenhao, conduct the physical examination, I'll perform the fasciotomy decompression. Time is tight, bleeding is substantial. Chen Dongmei, administer local anesthesia, prepare two debridement kits, stabilize the patient's right leg."
"Get the stethoscope immediately, bring over the bedside ultrasound," Su Wenhao called out as well.
While Li Tianyang had some reception experience, witnessing a fasciotomy decompression right in the rescue room was a first for him.
For Chen Xuemei, this was bewildering.
Local anesthesia?
She knew how to do local anesthesia, normally required when stitching patient wounds. But for a fasciotomy decompression? With a large incision area, how to administer anesthesia?
Just line the leg with injections? However, that could waste more time after it's done, right?
"Fool the patient, cut directly, just properly disinfect," Liu Banxia's words came to her ear amidst her dilemma.
That left her more dumbfounded; such an approach?
But Liu Banxia's words snapped her out of her daze, and she hurriedly set about disinfecting the patient's right leg.
"Don't worry too much; anesthesia is done, it won't hurt much," Liu Banxia reassured the patient as he put on gloves.
Then, with both hands, he took hold of two scalpels.
This maneuver startled everyone in the rescue room, even the working nurses showed concern.
Clearly, Liu Banxia intended to cut from both sides, completing the bilateral fasciotomy decompression at once. Yet, without any auxiliary examination, it was a highly risky method.
Many, even when fully focused, might fail at this surgery, and Liu Banxia was now attempting dual-tasking.
"The purpose of a fasciotomy decompression is to release trapped blood in the fascial compartment and relieve pressure," Liu Banxia explained.
"It commonly occurs in the arms and thighs, especially among patients with hemorrhagic trauma. Rapid treatment is judged primarily based on the patient's condition."
"You can see the patient's severe leg swelling, with skin discoloration emerging. Without timely intervention, thrombosis might form within the vessels, complicating treatment."
"Done, leg tourniquet complete. Cutting isn't that complex. Just make internal and external incisions parallel along the tibial axis of equal length," Liu Banxia concluded.
"Ah..."
As Liu Banxia swiftly cut through the patient's leg with two scalpels, one in each hand, blood from the compartment syndrome emerged, and the patient let out a scream.
Just after setting the bone, the patient endured it in a daze, but now these two long incisions were not so easy to tolerate.
"That's amazing."
Li Tianyang couldn't help but compliment.
It was because Liu Banxia's movements were so fast that the patient experienced a "delayed scream" phenomenon. More importantly, Liu Banxia was holding a scalpel in each hand.
With his current medical knowledge, he really couldn't tell which incision was longer just by eye; they both looked equally long.
Even if there was some difference, it would be at most a millimeter or two.
He felt a bit embarrassed after exclaiming, realizing his lack of composure.
His concern was unnecessary; nobody cared about what he had just thought, as everyone was in awe of Liu Banxia's dual scalpel technique.
It seemed so effortless as if Liu Banxia simply held two scalpels and made a swift cut. If it were him? Forget about making two incisions for a fasciotomy, he might not even be able to draw two parallel lines on white paper.
Not only did Liu Banxia manage to do it, but he did it so well. Dr. Liu truly is impressive.
In everyone's understanding, they hadn't seen anyone else perform such an operation; Liu Banxia was the only one they knew of.
"The auscultation results show diminished breath sounds on the right side, the abdomen is slightly firm. The ultrasound shows a lot of free fluid, unclear view, suspected spleen rupture, needs immediate surgery." At this moment, Su Wenhao, unaffected, also announced his examination results.
"Okay, X-ray the right leg and then immediately transfer to the operating room," Liu Banxia said.
"President Liu, is it done?" At this moment, Wu Mingyu walked in.
"Ready for imaging, everyone make way." Liu Banxia said.
"Ready, one, two, three."
"Snap"
"Alright, the reduction is quite good. Are you going to do internal fixation later?" Wu Mingyu asked while looking at the displayed X-ray image.
"We'll need to do internal fixation, otherwise the recovery will be slow, and it might cause secondary injuries, right? I also need to deal with his spleen; it might be shattered. Despite blood and fluid resuscitation for so long, his blood pressure hasn't improved," Liu Banxia said.
"Teacher Liu, I'll push him to the operating room now," Su Wenhao hurriedly said.
Liu Banxia nodded.
"Teacher Liu, how did you practice? I... I... I've lost faith in my future." Intern Xu Zhenkai spoke out everyone's thoughts.
"What's wrong? Learning from President Liu is a rare opportunity. How come you all look so demoralized?" Wu Mingyu curiously asked.
"Teacher Liu held scalpels in both hands and made simultaneous incisions on both sides," Zhang Hongfei explained.
"Damn, that's beastly." Wu Mingyu was stunned for a moment and couldn't help but curse.
"What's wrong? I just saw that he wasn't in a good state, and doing it separately would waste time and cause repeated pain, lowering his vital signs," Liu Banxia felt a bit aggrieved.
He really had no intention of showing off; he just naturally thought the fasciotomy should be done as quickly as possible.
Pain can cause significant harm to the body. If it becomes too severe, it could make the person pass out. And for the current patient, it's best not to faint, as blood pressure would drop even further if unconscious.
"Teacher Liu, did you lie to the patient about the anesthesia for this reason?" Chen Hongmei asked.
"It's a kind lie, I suppose. Although his thoughts can't keep up now, there's still concern. The pain of the incision is great, so it's best to keep him calm," Liu Banxia said.
"The patient's young, experiencing such an accident must be terrifying. His current heart rate and blood pressure are partly affected by abdominal trauma and leg fracture, and partly from being scared."
"All of you will join me in the surgery later. I glanced at the ultrasound earlier, and it seems we might have to remove the spleen. Just hoping there are no other bleeding points, the chance of a successful surgery would be better."
The interns nodded, though their minds were already somewhat overwhelmed.
Liu Banxia considered too many factors and in all aspects. While performing the fasciotomy, he could also observe the abdominal ultrasound Su Wenhao did on the patient.
As the orthopedic Teacher Wu mentioned, this is beastly. Is this something a human could do? Who can do so many things at once?
Just performing the dual-sided incisions together is already overwhelming.
The upcoming year, it seems, will be much tougher than expected. When there's time, I'll seek guidance from Teacher Xu; otherwise, it'll be hard to get through these days.
