Practicing Medical Skills in a Small Clinic-Chapter 53: Skilled in Medication Usage
Some biases are deeply ingrained in the bones of doctors and nurses in large hospitals.
This nurse isn’t malicious; you could tell from the way she reminded Li Jingsheng that night.
But her sense of superiority, along with preconceived prejudices, made her words uncomfortable to hear.
"Young lady, you can’t just say whatever you want! A few days ago, an old lady had an issue here, but it was her own heart attack. Dr. Li acted quickly, and it had nothing to do with their clinic. Today, the person who had an issue is Dr. Zhao from Quick Recovery Clinic. He had an attack just as he was walking out. Dr. Li has been doing all he can to help, and many people saw it happen. Saying it happened at Dr. Li’s clinic is really unfounded,"
Ms. Ou, having finished her IV, needed to stay for observation for another thirty minutes before she could leave.
Just in time to witness the whole process.
She’s now a devoted fan of Sunshine Clinic, and she stood up to speak out when she heard the ambulance nurse wrongly accusing Li Jingsheng.
The others buying medicine just watched coldly, with no intention of speaking up for Sunshine Clinic.
Most people, as long as it doesn’t concern their own interests, prefer to avoid unnecessary trouble.
Few are willing to get involved in others’ business.
It’s human nature to seek benefits and avoid harm, and to avoid getting into trouble.
Li Jingsheng really didn’t expect Ms. Ou to be so chivalrous and was quite moved by it.
"Sorry!" The nurse realized she misspoke, and gave Li Jingsheng an awkward smile. "Lend a hand, let’s get this patient onto the ambulance quickly! From the looks of it, he’s in a bad state, and needs to be taken to the hospital for further examination and treatment as soon as possible."
She switched focus to ease the awkwardness.
Li Jingsheng, along with Xiaogao and the ambulance doctor, first got Boss Zhao onto the stretcher, then all together, they lifted him onto the ambulance. 𝒇𝙧𝙚𝓮𝔀𝓮𝒃𝙣𝓸𝒗𝒆𝒍.𝙘𝒐𝒎
Seeing Boss Zhao’s short stature, they hadn’t expected him to be so heavy.
The three of them worked up a sweat lifting him onto the ambulance.
When Boss Zhao was standing, he just seemed a bit fat, with a businessman’s belly. But lying flat on the stretcher, his belly was round and bulging, even larger than a typical pregnant woman’s.
He probably lived well, indulging in grand meals.
Without exercise, he maintained a weight nearing a hundred seventy to eighty pounds.
"These are the vital signs we measured, and we’ve given him fast-acting heart pills and blood pressure medication," Li Jingsheng quickly jotted down Boss Zhao’s heart rate, blood pressure, and other symptoms on paper.
After handing it to the nurse, he added, "We gave him a double-dose of fast-acting heart pills, two pills, equivalent to six regular ones. The three antihypertensive pills were felodipine, 2.5mg each. Considering his very high blood pressure is likely primary and surged due to stress, I only gave him three pills, approximately a 7.5mg dose."
For felodipine antihypertensive medication, typically, 2.5mg a day is sufficient.
Many elderly patients with hypertension use calcium channel blockers to lower pressure.
Li Jingsheng’s decision to administer a 7.5mg dose was, after consideration, the optimal choice.
A blood pressure reading of 160mmHg is already alarming.
Hypertension often induces strokes and heart attacks, accelerating vascular sclerosis and significantly increasing the risk of vascular blockages or ruptures, while also harming the liver and kidneys.
It’s immensely harmful.
Overuse of calcium channel blocker antihypertensive medication can damage the liver, and Li Jingsheng took a lot of care when medicating Boss Zhao.
"Buddy, I have to say, your understanding of drug efficacy and pharmacokinetics seems quite good!"
The ambulance doctor couldn’t help but praise Li Jingsheng.
Inept as small clinic doctors are often stereotyped, his proficiency might have been shocking to him.
After all, many clinic owners are seen as doctors who’ve been ousted from formal hospitals.
The township health centers and community health service stations are considered formal hospitals, albeit small ones.
But for a doctor to successfully run their own clinic, they must have some skills. Those lacking understanding couldn’t sustain a clinic for long before it would fail.
Regardless, the skill level of clinic doctors is deemed low in the industry.
That’s why the ambulance doctor was surprised to see Li Jingsheng’s proficiency in drug efficacy and pharmacokinetics.
"Ha, you’re too kind! I only know a little bit,"
Li Jingsheng modestly replied.
"Your proficiency in antihypertensive medication is quite high, don’t underestimate yourself. At least, I think your skills are comparable to senior resident doctors in our Cardiovascular Medicine department. You probably treat a lot of hypertensive patients and have developed solid expertise. I think focusing on these areas is great,"
The ambulance nurse praised Li Jingsheng as well, perhaps as a subtle apology for her earlier rash comments.
Her words still carried a hint of disdain for clinic doctors.
She didn’t believe Li Jingsheng was proficient across various medical fields, only in antihypertensive medication.
Li Jingsheng had no intention of debating with her.
He simply nodded slightly at her, then jumped off the ambulance and said to Xiaogao, "Escort your boss to the hospital, I still have the clinic open for business and I won’t be going."
Xiaogao was still young and a bit naive entering society.
Li Jingsheng reminded him, "You should call Boss Zhao’s family, have them come to handle any advances for medical bills and signatures."
Boss Zhao isn’t a good man, and his family is likely not easy either.
Should Xiaogao advance medical fees and not get reimbursed, he’d be in trouble.
"Got it! Thank you!"
Xiaogao nodded gratefully.
Watching the ambulance leave, the crowd began to disperse in small groups, discussing among themselves.
"Dr. Li, do you have time now? I’m quite interested in the banner you received, and equally interested in your clinic. To be honest, my two visits to your clinic have changed my view of clinics as basic healthcare institutions frequently facing issues. I’d like to interview you in depth, hoping to do a more comprehensive report to give the public a better understanding of clinics, so they can have more faith when seeking medical care or purchasing medicine."
The female journalist was persistent.
She wanted to uncover more newsworthy details from Li Jingsheng’s clinic.
Her bright, intelligent eyes looked at Li Jingsheng with expectation and pleading.
Such an opportunity to be featured in the media, most clinic owners would love to pay for it.
Moreover, a beautiful and intellectual lady reporter was looking at him with a request in her eyes; few men could refuse.
Becoming famous and getting more chances to interact with a beautiful journalist—Li Jingsheng was single, after all.
Most would enthusiastically agree.
"Sorry, I still have customers to attend to and patients to see; I really don’t have time for an interview. There are far more outstanding clinics than ours. For instance, Quick Recovery Clinic on this street is ten times busier and much larger, more typical. Jinkui Clinic is also excellent. If you’re interested in clinics, I’d suggest interviewing those two."
This seemed like redirecting trouble elsewhere, didn’t it?
Having been duped by Boss Zhao multiple times, Li Jingsheng had learned some tricks.
After speaking, he no longer paid attention to the charming female journalist.
He went straight to serving customers.
Tang Ping watched her boss ignore the reporter and photographer, secretly accusing him of foolishness.
Such an opportunity for fame was something money couldn’t buy!
At times, when her boss was stubborn, it was truly frustrating.
But what could she do?
Employees have to listen to their boss, after all!
