Chapter 2013: Meticulous Craftsmanship

Doctor Hu expressed his concerns after listening: "The blood vessel is only so big. Wouldn’t injecting blood flow cause it to burst?"

Small blood vessels are very prone to bursting, which is also why ophthalmologists are hesitant to perform interventional surgery on her. An embolus lodged in the small blood vessels, forcibly injecting too much pressure can easily rupture the vessels. At that point, don’t even think about thrombolysis; the eye may be blinded directly.

"Therefore, attention needs to be paid to the possibility of hemodynamic supply from the surrounding collateral circulation to this part of the arterial network, as well as the anatomical position of the blood vessels," Xie Wanying said, enhancing the vascular diagram drawn by Senior Cao for the family, "The ophthalmic artery comes from here, this is the cavernous segment of the internal carotid artery. After emerging from here, it has to pass through the dura sheath and then enter the cranial cavity. The ophthalmic artery is also the first branch of the internal carotid artery. It travels from below and laterally to the optic nerve, crossing internally, and gives off the central retinal artery (CRA), which is the temporarily judged location of Teacher Hu’s condition."

Mr. Liu leaned in closer, watching her hand move like an artist, no—a design engineer, drawing with such precision that it required no modification, truly a masterpiece of technical finesse.

He believed Cao Yong could draw like this; after all, he is renowned as a neurosurgery expert. But as far as he knows, Dr. Xie is just a medical student. Mr. Liu was entranced, noticing the more she drew, the less difference he saw between her drawings and Cao Yong’s.

Cao Yong, perhaps, as a great expert, made a rough sketch for the family outlining the essentials. Xie Wanying’s drawing seemed like adding various details and coloring to a sketch base.

It can’t be said that framing or the subsequent detailing is more impressive; both require technical skill.

The latter drawing was detailed and beautifully clear, which is indeed remarkable.

"In fact, the retina has two vascular networks, one part supplying blood to the internal retina called the retinal arterial network, while the external network is supplied by the choroidal vascular network to the external retina. The central retinal artery is divided into three segments. Firstly, the intraorbital segment extends from the origin to the dura sheath around the optic nerve. Next is the intradural segment, located between the optic nerve and the dura sheath. The intra-neural segment is within the optic nerve. The junction of the central retinal artery and the retinal artery branches is here, at the optic disc." Xie Wanying pulled out a red pen to circle the key points.

Mr. Liu was captivated, proving that anatomical diagrams, when drawn beautifully, are as attractive as art taught at the Academy of Fine Arts.

"For some people, their retina is supplied by the ciliary retinal artery, a branch of the posterior ciliary artery. Therefore, having Doctor Hu thoroughly examine these vessels is crucial to understanding where the problem specifically lies. At the optic nerve head, the diameter of the central retinal artery is about 160 um. There are several narrow spots on this artery; one is where it passes through the sieve plate, and another is at the proximal end where it enters the optic nerve through the dura. These narrow spots might be where we look for the embolus, as emboli are most likely lodged in these places."

"Once the embolus is found, how do you propose to flush it out?" Mr. Liu was already following Xie Wanying’s proposal without hesitation. 𝗳𝐫𝚎𝗲𝚠𝚎𝗯𝕟𝐨𝘃𝚎𝗹.𝗰𝗼𝗺

Doctor Hu could tell that her husband understood clearly and was deeply interested, so she couldn’t help but smile. That said, Student Xie indeed explained the anatomical diagram very well. Now, it’s up to Student Xie to explain how to tackle the critical issue and the key strategic solution.
